Texas PTA Honorary Life Membership Award:
This is an honor bestowed by Texas PTA. It is awarded to individuals who have
made outstanding contributions to the welfare of children and youth. It is one of
the highest forms of recognition that demonstrates to volunteers how much they
are truly appreciated.

Texas PTA Extended Service Award:
This is an honor reserved for people who have previously received the Texas
PTA Honorary Life Membership Award, yet still continue to make
outstanding contributions to the welfare of children and youth. This individual
must have been involved with PTA for a minimum of 10 years.

National PTA Life Achievement Award:
This is an honor given by the National PTA. This highest honor is given to a
person who daily lives out his or her commitment to children and acts as a
tireless advocate for children’s education, health and well-being.

(in a sealed envelope) marked “PTA Life Membership Award” or you may email your nomination to the PTA/PTSA Life Membership Chair ([email protected]). Be sure to include a summary as to why you are nominating this individual; you may use the back of the form or include a separate sheet. It is not necessary that this person be a member of the Canyon Ridge PTA. The deadline for these nominations to be returned is Tuesday, November 16, 2022 . The chosen award recipients will be honored at the NEISD Council of PTAs Founders’ Day celebration in February 3 of 2023.
